If you have never had a child attend Storm Lake Community School District before, you will register through our kiosk portal (Click Here) or you can visit us at our Central Administration Office located at 419 Lake Avenue in Storm Lake to complete the registration process on one of our kiosk computers. Before you begin, please gather the following:

Household Information -- Address & Phone Numbers
Parent Information -- Cell Phone Numbers, Work Numbers & Email Addresses
Student Information -- Demographic & Health/Medication Information
Emergency Contact Information - Addresses & Phone Numbers

The Storm Lake Community School District requires a copy of the following prior to enrollment:

Birth Certificate, Passport OR other official document can be used to verify the student's legal name. (All documents will be automatically deleted upon verification of spelling. No documents will be saved or stored.).
Immunization Record
Transcript (9th - 12th Grade ONLY)
You are able to upload all required documents into this application or you may turn in a copy at the Storm Lake Community School District's Central Office at 419 Lake Avenue, Monday-Friday, from 8:00 AM - 4:00 PM.
